First Visit to Our Office
Please arrive 10-15 minutes prior to your scheduled appointment time. Please bring the following items with you to every appointment:
- Current list of all prescription and over the counter medications with name, dose and frequency of each medication.
- Insurance card
- New patient paperwork packet (available for print on our website here)
How We Return Your Phone Calls
We are with patients during clinic hours and are unable to take most phone calls directly. Our receptionists are trained to obtain as much information as possible regarding your issues so that the nurse is able to speak with the physician prior to returning your call. Please notify the receptionist of urgent issues for a more prompt response.
Prescription Refills and Prior-Authorizations
Please contact your pharmacy first. They will send the refill request information to our office for approval. It is necessary is some cases to see the physician prior to refilling certain medications. Please take care of medication refills during the week. Refills will not be called in after hours.
If your insurance policy requires prior-authorization for your medication, you will need to contact your insurance to have them send the prior-authorization form to our office. Our fax number is 806-355-9989. You may also visit www.covermymeds.com to see if your plan/medication information is available.
If you have a medical emergency you should go to the emergency room of your choice so that immediate care may be given. Quail Creek Ear, Nose & Throat Center is not an emergency treatment facility.
If you need to speak with your physician after hours (Monday through Thursday 8:00am -5:00pm, Friday 8:00am to noon) the answering service will take your information and contact the on call physician who will then return your call. This service is for non-routine, urgent matters.
